Do I need to arrive early for my first appointment?

In order to be completely prepared for your visit, please go to the bottom of our website, and click on the patient forms button to print out your patient registration, medical history, and dental history forms. Please complete these and bring them with you to your first visit. If you don’t have a chance to complete your forms ahead of time, please come 20 minutes early, and complete them in our office.

 

Do I need a mouthguard?

Every year over 5 million teeth are lost due to sports injuries (soccer, football, basketball, hockey, lacrosse, wrestling, biking, skating, and other activities). Wearing a properly fitted mouthguard can protect you from losing a tooth or teeth. There are two basic options for mouthguards.

  1. You can go to the drugstore, and purchase a “boil and bite” mouthguard, which may be bulky and uncomfortable to wear.
  2. You can have us make a custom fitted mouth guard that fits well, and is comfortable to wear.
Does the dentist work emergency hours?

Yes! We reserve time every day to see our patients who may be experiencing an emergency. During regular office hours, please call us at 317-841-3130, and we will find a time to see you. If you are experiencing an emergency after hours, please call our after hours number, 317-625-5010, and leave a message. We will call you back to find out how we can help.

How old should my kids be before their first visit to the dentist?

The American Academy of Pediatrics recommends your child be seen by a dentist for a routine cleaning by their first birthday, to be sure everything is progressing normally. At this first “get acquainted” visit, we also consult with the parent to give them the best advice about how to care for your child’s teeth.

  • The fee is for a 12-month period and is due, in full, upon joining

  • Membership is effective the first of the month in which payment is received

  • The anniversary month of your membership is the month you originally joined regardless of when payment is received. Renewal payment, if elected, is due on the first of the anniversary month each year

  • It is the responsibility of the member to maximize membership by arranging all appointments within the 12-month membership period (if appointments are not used, the member will not be entitled to a refund)

  • When applicable, all payments are due at the time of service

  • Membership fees are subject to change on an annual basis
